One addition i have to make is splatterhouse on the GENESIS .. that game was insanely tough.. especially the part where u have to save the hero's wife before she gets infected

and maybe animaniacs.. but that was prolly coz of horrible level design

Ghouls N' Ghosts was a very hard game.  It definitely belongs on the list.

Ghouls N Ghosts is indeed difficult but really doesnt deserve the second place , i believe at the time i played it Ghost Busters on Genesis was more difficult. and i think Ninja Gaiden should be third and Battletoads should be first.
