Many infractions you will recieve a warning before any ban, but there are a few where moderators have been specifically granted discretion to bypass a warning in favor of a ban where warranted.  Most often it is applied when one member directly insults another member.

I got a $2000 fine and recently got my account freed up.  Must be because of the holidays.
