How does one get double perma banned? They get permabanned, then asked back, THEN DENIED!?!

I thought it was just issuing two permabans at the same time kind of like a double life sentence for prison.

in the past few months we noticed that a few previously permabanned users were suddenly not permabanned anymore, so we hit them again.
We're not quite sure what caused that.

Some even got 3 permabans in a row... maybe even 4.
