The last time I read a walkthrough was back in 2006 for FFXII.
I used walkthroughs for FFX and God of War. Game walkthroughs helped me get preety far in Final Fantasy X.

Yeah for things like GTAIV Pigeons, Feathers in ACII and Nuke Cola in Fallout 3 ..
Usually on the 2nd playthrough, for sidequests etc. And I print out fusion rules for SMT games.
Sometimes. I used it for God Of War a few times when I beat it last week. I used it when I play FFXII over the last two years (I'm slowly getting there). I'll generally only use it if I am started to have trouble with something and it is making the game sluggish and unenjoyable I'll use it to get to the next step real fast so that the game can pick up pace again.
Rarely, I try to do without. But sometimes figuring something out on your own just isn't worth the frustration.
I'm really trying not to. I used it for Metroid Prime and seriously regretted it; the puzzles and overworld were so well constructed for a person trying to play with no knowledge and I bypassed all of that...
I'm waiting for a good new exploratory (Nintendo) game that I can enjoy without using a walkthrough. Since the Wii launch there's only been SMG and NSMBWii, which were good but too easy to need a walkthrough for anyway. I'm hoping SMG2 or Other M or a new Paper Mario at a long shot would fulfill this.
Only if im stuck for good, or when a puzzle proves to be ridiculously hard and i have little time. So it happens but not too often
I only do if I am about to throw the game out the window.
