Friendly fire was on by default in the beta. I have no idea why they changed that for the final game. I always turn it on when creating a game.
Trust me, you don't want FF on. Why? n00bs can't tell the difference and the vast majority of your deaths would be thanks to teammates. Take it from a very experienced kz2 player.

If FF was the norm, people wouldn't be so surprised when they join a game with FF, and this wouldn't be a problem. =/
Because it's fucking annoying.
1. People always run into nades and get in the way of people shooting.
2. Assholes will join for the sole purpose of killing their team.
3. KZ2 is set up so it is impossible to actually kick any of these people
4. Saboteurs would make everyone paranoid
When I make a room, I always have friendlyfire one. Having it off is for casual :P

Because then random rooms wouldn't be choke point cluster fucks for 95% of the game, and some thought and tactics would be required, which is beyond the capabilities of the general public.

It's not the element of surprise. Many players know and exploit it purposefully.
Some would kill you just to revive you (I think the dude was working on the medic badge). Others kill assasination targets for the lulz. Others don't really care and still toss random 'nades into crowded s&d zones.
I saw a guy finish a long game with only 27 points because he was over 100 points in the negative.
The biggest thing however is that unlike suicides, deaths from FF affect your K/D ratio. It's kinda weird to have a low K/D because you were killed by your own teammates.
The only benefit of FF is to stop rocket spamming but frankly they're not that hard to deal with anymore. Assault guys don't regenerate body armor once it's gone and the armor is only 50% extra HP now.

BTW, FF is no problem for clan games where everyone knows what they're doing but you're better off having it off when joining 32-player public games.
