1) All the weaker varients were wiped out, leaving only the ones that were always stronger to have a monopoly on victims. For example, there are 2 groups of dogs living somewhere (for lack of a better place). One group is short-haired, the other long-haired. One year the temperature drops, and the short-haired dogs die, and only the long-haired ones are left.
2) Random mutation of a certain strain allowed it an advantage against anitbiotics (and I'm going to assert here that random mutation does not equal evolution). For instance, a certain bacteria experiences a mutation that disables it ability to produce a substance that counters penicillin, and thus begans producing huge and uncontrolled quantities of this anti-antibiotic. Now, penicillin can't hurt it. On the other hand, the bacteria is now producing massive amounts of this stuff and is potentially wasting resources in the event of an absence of penicillin.
