It's tough to say. Old arcade games were not "hard" so long as you had plenty of quarters. And even then, they just threw hordes of enemies at you so you WOULD pump in more quarters. I'd even say some of them were impossible to beat without getting hit a few times.
And many old console games were hard due to bad level design.
Battletoads, which I love, was ultimately an exercise in memorization. I'm not "better" at the speeder level than I used to be... I just know where all the walls are.
So, to sum up, there is no real answer. I'd say the most "challenging" opponents have always been human. I don't count the computer cuz it can cheat 
Completing Tetris or Arknoid requires real skill that make people who complete Super Mario Galaxy, Halo, Call of Duty, Metal Gear Solid etc etc look like they have just completed a 4 piece wooden jigsaw puzzle.
I'm always very impressed watching people who can think and respond in a split second whilst playing such games.
I think I've mentioned these games before, but the hardest games I've played (and not beaten, by the way) are Bubble Bobble and The Hitchhiker's Guide To The Galaxy. I'd also add that the original Civilization is pretty damn hard when you play it on the hardest level with the max amount of enemies.

Completed Robocop in the arcade with one coin although it's so long ago I have no idea in how many lives.
When playing arcade games I only considered it completed if it was done in 1 credit.
But some games I did complete in 1 life in the arcades that are on MAME are:

Hitchhikers Guide to Galaxy isn't hard, it's bullshit.
Forgetting to get one item in the beginning of the game and then not needing it until the very end and if you didn't get it in the very beginning there's no way to win is the very definition of bullshit.
