It sounds very violent -- and a lot of fun.
This is another game that I am glad that I have the parental controls in place on my Wii (I have a 6-year-old son who does not need to be playing this game or NMH or several other titles I have).

I think that Manhunt 2 ran into problems because it was trying to be realistic. Madworld is more stylized and cartoonish. I thing that the ESRB will probably go easier on this game.
I think that we would have already heard something if they had to tone it down. This game releases in just a little over a month. We heard about the Manhunt issues several months before release IIRC.
