RE4 had several Holy Shit moments, even though I had been exposed to spoilers. The first time I fought "IT" (you meet him on those suspended railcars or whatever in that canyon) it was freaky as hell. And Salazar's Right Hand was really scarily cool.

Not so much in a game, but during one...
I was playing Bionic Commandos on C64 with a mate and it was the final level with Nuclear Warheads or some such (this happened about 20 years ago, so my mind is hazy) when the nuclear alert siren went off in town. We both bricked ourselves badly, switched over to BBC1, having paused the game, realised the England cricket match was still on and that we weren't about to be destroyed by the Russians, and went on to complete the game. FFVII - Not a WTF exactly, but when Cloud makes his speech about how he doesn't care about saving the world and just wants to kill Sephiroth, I thought it was pretty cool. NOt so cliched.
ffviii-when odin uses zantetsuken on seifer and it completely fails and seifer kills him. talk about a wtf moment
god of war 2-the ending sequence, such a huge cliffhanger that just left you sitting there goin I WANT MORE

MGS1- physco mantis fight
MGS4- believe its the end of disc 3, but when ocelot just beats the royal piss out of snake. I almost thought snake was gonna just bite the dust in the middle of the game.
eternal darkness- The first time my limbs just started randomly popping off
gears of war 2- finding maria, was the one really touching scene out of the whole game. wish epic put that kinda effort towards the whole game.
