Can't say 100%, but I've never been disappointed by Level-5, Atlus, or Naughty Dog. Square-Enix would be close, but they've really dropped from where I remembered them.

Yes and no.
There is always room to make mistakes, but I never doubted that I had to have Quest for Booty.
There are companies who will always excite me with new games, Insomniac for example.
