Relative to content per (whatever unit of measurement you want) I thought Fallout 4 pretty much nailed it. It still had open swamps and forests to explore but there was always something interesting nearby. You usually had a pick of landmarks you could head toward. If you ventured into the city or a few other areas then you could literally see different enemies to fight just by turning in a circle. In fact, this led me to discover that I really, really love instigating different factions into killing one another. For me, it made it a lot more fun game than Skyrim or New Vegas in terms of just wandering around, which is the reason I play these types of games in the first place.
