Number 5 is ALL wrong. It was none of those. NEC was.

TurboGrafx16 or PC Engine is the VERY FIRST console to EVER use CD-ROM.

Philips' CD-i came out in 1991, while the CD-ROM add on for NEC's PC Engine (AKA TurboGrafx-16) came out in 1988. ^_-

He does says "for their video game console". So even if there was a disc Zelda game for the CD-i, that console was from Philips, not Nintendo. And the aborted Playstation from the early 90's (which was made by Sony) doesn't count since there wasn't a game released for it, anyways.
