i'm going to go full on spoilers so if you haven't finished every MGS game up to this point then do not read post.

Kaz Miller is killed by Big Boss

Paz is not dead.

Ishmael is Paz's son 

Eli is Gray Fox / Frank Jaeger.

"Phantom Pain" is the feeling you get from losing a piece of your limbs and not got used to not having it there IE "Phantom" feeling of pain that isn't there, Big Boss loses his arm.

Kaz and chiper (Major Tom) is using Big Boss for their goals , Big Boss then turns into a "Demon" "Serving in Heaven (outer heaven)" but he goes out of his way and kills Kaz and then Major Tom (in MGS4) turning his Outer Heaven back to Zero "Better to reign in hell than serve in heaven" - Paradise Lost 

Moby Dick Project is Pacifica Ocean's (Paz Ortega's real name) creation and her son Ishmael to resurrect Ahab (Big Boss) from the dead, in order to kill Kaz and Cipher and the guns of the patriots.

Kaz means Destroyer of Peace, Paz means Peace, Peace mark is "V for Victory" Paz wants victory against Kaz and Cipher (Like Liquid Snake did) , and she gets it. V is not for Peace, but for Victory

Kaz can't get killed by Big Boss. o.O

He gets killed by Liquid.

EDIT: Kaz actually advises Solid Snake when fighting against Big Boss in Outer Heaven. He calls him "a monster".

Kaz helps Solid Snake to defeat Big Boss in Metal Gear 2 : solid snake, obviously leading him to burn big boss "killing him"  but he was resurrected (like a burned demon from hell) later with nanomachines and then killed Kaz for revenge (before the events of MGS1) and Zero in the end of MGS4

Or... maybe Liquid killed him in orders from Big Boss, and then stole his identity and used Solid Snake to get down the patriots (which is what happens in MGS4) 

either way Big Boss or Liquid killed Kaz.

also a little offtopic , someone took Big Boss' arm off so he used Ramon Galvez's red Ciggarete lighter Arm in this picture afterwards, which formed "Diamond Dogs" 

Damn... gotta stay off the net then  

but I'm guessing its called Ground Zeroes because Big Boss divided by Zero like he said in the end of Peace Walker.

I'm going to go back to this thread when the game comes out.... however i'm not going to be getting a PS4 =/ why aren't they releasing it on PC?
