There are three things that got announced. Unity Pro Tools for Xbox One, the Unity Tools Add-On for Xbox One, and the Unity Tools for Windows 8 and Windows Phone 8.
The latter two are free and for those that already have Unity Free or Pro.
The former is a free version of Unity Pro tailored specifically to development on the Xbox One.
The benefit of the Add-on for Windows is that it allows a develop to "port" their code to the Windows store. The Xbox One version of Unity Pro and the Add-on for Unity provides pre-baked functionality.
So, to recap.
a. Unity Pro for Xbox One is free to developers, as well as the Add-On for Windows and Xbox for existing licensees of Unity Pro and Unity Free.
b. Yes, it does, but you actually have to read the official statement, "...as will special Xbox One-only Unity Pro seat licenses for Xbox One developers in the ID@Xbox program."
c. They talk about both. Reading is fundemental.
