I'm going to hell for making fun of such a terrible accident, but...

... and I would hate to be that engineer. His guilt trip has got to be immense.

Not the engineer's fault.  He did everything right.

The problem is, the engine had a fire.  The fire department responded, and turned off the engine in the process.  The engine powered the air brakes, and lacking the engine to maintain air pressure, eventually the air brakes released.  The hand brake, was also engaged, but it has yet to be determined if those were disengaged or failed.

Both the fire department and the train company would seem to be responsible.  The fire department didn't wait until someone from the train company arrived to ensure the train was safe and secure, and no one from the train company arrived to sure the same.  The fire department left, and not long after the train was seen rolling down the tracks toward the town.
