Interesting. I don't remember a 2nd difficulty level. I have beaten the game though, it was rather difficult. But games back then were difficult unlike modern games.

If you look at the stage #, instead of saying "1" it says something like "Stage 19" (not on my Wii atm). |
I've beaten the higher difficulty once about 3 years ago. I got the VC version for the Wii. It's tough, but doable.

CVII is a really good adventure, but it is absurdly easy. I've only ever finished CVIII once, though I have beaten akumajo densetsu (CV III jap) several times. Those games being as hard as they were was a misconception from the Japanese developers believing that those games were not hard enough for American audiences so they jacked up the difficulties on CV I and III and also did that with the Ninja Gaiden series (the Japanese versions are much much easier.)

Honestly, I never played the game a second time through because after beating the first run I'd had enough! :P
I think it's a pretty difficult game (like Ghosts 'n Goblins) but not ridiculous like, say, R-Type 2.
