I think you should add Sorceress Adel (Final Fantasy VIII) to the list, after talking with some other friends who have played the game they think she is an excellent villain.

One of the only villains in a Final Fantasy game to make me crap my pants.

I don't know what the most scary part about her, the fact that before she was frozen, not a single person, let alone an army would dare lay a finger on her or the fact that she is 10ft tall and looks like a man.
