The best way to make sure you get it is to load up the already completed game and change the difficulty to hard and then save and exit.
Start a new game and the difficulty should be on hard by default.
I used the same method for both Infamous 1 and 2.

As soon as I got the game and started it up I switched to hard mode the moment I was able to control Cole and I got the trophy.

Yup, this is what I used to get my platinum trophy
....and for your other question, hard isnt really that hard. Its harder than normal but nothing too crazy.
