Aside from that, I think Guilty Gear Isuka (and/or Accent Core?) has a 4 player mode, and Jump Ultimate Stars has 4 player online. You're definitely best off with Smash though. =P
Power Stone for DC, such a great game, shame I only got to play it like 5 times round at a friend's house.

Super Smash Bros., Super Smash Bros. Melee, and maybe even Super Smash Bros. Brawl. The Super Smash Bros. franchise is the best fighting game for 4 players! At least I think so.

Powerstone I or II for the Dreamcast. Two is a bit more party oriented. In fact, I don't even remember if one has four players simultaneous.
There's also a lot of games with tag or "team" modes where you swap which player and/or character is in but don't have true multiplayer simultaneous combat.
Also, though the Worms series aren't actually fighting games (though they have 2-D gameplay and hp =D), they rock for competitive, fun, multiplayer. Instead they are a turn-based yet realtime, 2-D, wtf is going on, strategy game.

Yes, both Powerstone games had 4 player simi play.
Dead or alive 2 for Dreamcast and PS2 (with multitap) allowed for 4 players, although it was only 2 at a time.
Clay Fighter 63 1/3 for N64 also had 4 player tag style multiplay.
Almost all Wrestling games are 4 player, although it's not really a fighting game.
