But i doubt that you can do all the career's in one Playthrough.. no matter how good the game is, finishing it 5 times... nah, i like to invest 200 Hours into 1 Character, instead of splitting it over several. Even if there is no real 'additional time' by doing so. :/
(For Example Demon's Souls: i only had one Character to the end, and the same, year back with Phantasy Star Online. Whitill forever lol)

i doubt they gave it more than the second, since they said that TW2 is an IMPROVEMENT. :>.. and as far as i know, TW1 didnt receive any Ratings above 85%. ^^

I dont think you will need to do that. AThe games is very much cast-less like Oblivion. So you should be able to do every questline.
As for the use skill x times, I think I heard somewhere that this game will let you pay to rearrange your skills. Im not sure though.

Mirek Dymek: He has the Choice, but not the Pain. :) (<-its a german saying, so translating it is stupid, he's requesting for dunno... 'The Agony of Choice?' no idea.)The Player can basically become Member of any Guild, so you dont have to commit to a certain one. But there is a cap for how far you can get in the Single Groups, So for Example, your Traders Career will be over, once you did assasinate their leader for the Thiefs Guild. And of course, at a certain point of your career, you'll have access to Quests and Items that are kept away from Non-Members.
