I don't see how killing being required in some portions or killing existing in the game is a refutation of "Killing is supposed to be a last resort". It was precisely because it was absolutely required, that Lambert lifts the restrictions on Sam, and gives him the Fifth Freedom to kill the Georgian President, to kill Shetland in those situtations.
And from the clips I saw of Grim, it's something like:
Sam: "Why are you doing X?
Grim: "Shut up and do what I say"
Sam: "Tell me why"
Grim: "Shut the fuck up!"
While she's doing some pissy looking pose, dressed up like that girl in MGS4.
