It would need it's very own assets (simplified foliage, for instance) to make it run, and probably would have late pop-ups due to the CPU limitations, but with such deeper changes in mind, there's no reason it would be impossible. Plus, Witcher 3 code is actually not very optimized, maybe that can be optimized a bit while porting.

For instance, TW3 reaches 12(minimum FPS) -15 FPS (max) on such a lowly machine as an A8 5550M on low preset and post-processing in 1024x768. which should fit the Switch's capabilities both on CPU and GPU in handheld mode pretty well. Simplify a few assets, constant map loading with pop-ups and variable resolution and 30fps in something like 840x480 in handheld mode and 720p (both median resolution, drops below that would happen, but mostly be in those resolution) in docked ain't impossible.

If even base PS4/Xbox 1 sometimes struggled to keep 30fps, Switch which is 4-5 times weaker than those systems, no way it can achieve 30fps, more like 20fps at most

Honestly it depends on the port team and budget. They got rise of the tomb raider to run on 360 and look pretty damn good, probably one of the most impressive ports  ever. of course this is  much more difficult Job. I feel like they would have to make the game from the ground up.
