I entered Honest Hearts (Fallout New Vegas) with the 'wrong' apparel and everyone was attacking me, by the time I realized my mistake I had no desire to restart so I never played it 'properly'.
Deus Ex (2000) - a game that pushes the boundaries of what the video game medium is capable of to a degree unmatched to this very day.
