For the people that don't know, you *can* get free health care in America if you are poor. If you aren't poor, then you have to pay. However, many people (myself included) have jobs that include insurance as one of their perks. I can't imagine there are a whole lot of people who actually pay out-of-pocket for their health insurance in America. You really have to look at the big picture. If you live in a country with free health care but a low median income, then the quality of life isn't as great as it might seem.
