With mouse and keyboard most of the time. But when I feel like it, I play also on the TV, so a xbox360 controller is a must.

And yes, it's 30 fps most of the time, because of my PC specs. So I don't bother with it much. But you can check some youtube videos on how doom performs in 4k on a gtx 1070.
