It depends on a number of factors. If you're doing an HD remake, it's a job for an intern for a couple of months to put in the hooks, and make sure they get triggered correctly since this will undoubtedly be built into whatever engine is running off the PS3/PS4/PSV. If you're just emulating the original game on an emulator running on these console, then you're going to have to modify the emulator to support trophies, then you're going to have to modify the game itself to be able to interface with the trophy system, and trigger the trophies. This might not be possible if you don't have access to the source code.
