Marketing was terrible. Name confused consumers who to this day don't know what it is. Gamepad was neat but lacked broad appeal like motion controls and drove up the price of the system, causing it to be weaker than it should have been and more expensive than it should have been. The relative weakness of the hardware combined with initial low sales to push third parties away. And Nintendo screwed up by having one of the worst droughts ever post-launch for the first half of 2013, killing any momentum the system could have had.
I'm reminded of the scene from the Producers where Zero Mostel's character lists all the wrong choices he made and wails 'Where did I go right?'
