Fallout 4 has a lot of content, yes, but that content (as in places and objects and NPCs, etc) has to be shown on screen and with polygons and textures, and the game is lacking in those parts which is the reason of why it looks bad compared to other modern games like The Witcher 3 that is also a WRPG.
The next gen version of GTA V is not just an upscaled port, it has improved geometry, textures and... well, basically everything that could be improved, was improved. And those 50GB are for the console versions, at least the PS4 one.
