It's also clarified in the Dam chapter that Joel raised Tommy by himself and struggled with it.
Now it's obvious why he clung to Sarah so much and was so bitter and closed after her death. Everyone in his life died or left him, just like everyone in Ellie's life died or left her. Deep huh?
