I already gifted a game to someone who didn't have a GOG account at the time. He had to create an account but after that everything worked fine

I didn't buy the game as a gift (I got the game as a gift from GOG, but I had it already in my list), so it's possible that you will get the mail and have to resend the mail or at least the code to the respective person. But the code should work with everyone, provided the person you want to gift the game doesn't have the game not on their account. If they got no account yet, there shouldn't be a problem.
