Well, he shows a lot of poor decision making and time mismanagement. You don't have your composer write a bunch of music that doesn't get used and you don't delay your game due to engine tweaks. He DGAF about getting the project timely and that's what got him canned.
This is what separates gaming from other business as it's creator driven, not profit driven. Kojima fine tuned his product to fit his vision which is definitely admirable, but there has to be some compromise.
