Batman and Spawn have fought but neither 'won'. Batman would probably win.

Batman does beat Superman for one very simple reason. Superman is a good guy. His own instincts mean he doesn't kill or gravely injure, he holds back because of his power.

When you are against a guy who won't to make sure he wins... that's a problem.

Dr Manhattan is a reality manipulator, better than most as he seems undefeatable. It's not a matter of what Supes can withstand but if he can beat Manhattan and he just can't. Besides, if you remove someones head...
