From your list, numbers five and eight are the most bothersome to me.

For number five, it's frustrating when a few unused party members are doomed to lower experience levels. Now, number eight is frustrating also, although it's not unique to role-playing games. Missing some essential conversation or location can be devastating, especially if it sets you on an unwelcome path. I remember I said the wrong thing at the wrong time to Leliana in Dragon Age and missed out on a romance. I'm still bitter about that
