Experimentation, dungeon design, puzzes, art style, what kind of world, that kind of issues. A lot of thought goes into it before they even start, I think that is the difference.

GTA takes a lot of work as well of course, but in a different way. It's more like they pick a city and have a huge amount of people trying to recreate it ingame. Takes a lot of time but is more 'doing' and less 'thinking'. And they probably have much larger team working on it as well.

I wouldn't know for sure of course, but that's what I think.
