Same! I've never even thought about killing the other two. Just the one that's about to do the surgery, then grab Ellie and leg it.


I guess it's due to how the hospital level was handed in term of story and gameplay. Throughout the game, you can sneak around the areas using stealth and minimum killing. But in the hospital level, the game is forcing you to do what was basically a cleansing of the fireflies as way to show how Joel will do anything for Ellie, which means no one will be alive because "you'll come after her" in his own words to Marlene. By the time I reached the operation room and killed the 1st doctors, I felt something was wrong but it didn't hit me until the nurse said "you're a fu*king animal" and later begging for her life then I realized the truth which I found it hard to digest no matter how I tried. Joel isn't better than the other "bad" guys in the game. He is survivor, and will do anything to be that way. 


Well at thay particular time in the game, Joel IS the bad guy. If you really paid attention throughout the story, Joel KEEPS losing the people around him that he cares for. Something obviously happened to his wife, his daughter was.. you know, him and his brother had a falling out it didnt see each other for years until you ran into him at the damn( which you then see that the brother has a wife a potential family he has to look after), and the only person he was with that he cared for when they ran into Ellie was Tess. 

 

Throughout the game you can see that Joel avoids "caring" for people because of this. Then while they're taking Ellie to the fireflies, Tess, the one and only person  he could care for gets "taken away" from just like everybody else. Then he was stuck with some random kid. By the end of the game Ellie became someone that he was starting to care for no matter how much he tried to avoid it. When you were near the firefly hospital right after you killed that David guy who was trying to fuck Ellie, Joel had pretty much opened up to Ellie at that point and told her that he wasn't going to leave the hospital without her. So after this long journey with this girl who he had finally started caring for, they get there and Joel finds out that this whole journey was basically for Ellie to die.

 

So this time, the VERY LAST person with Joel that he cared for was about to be taken away from. Joel only had 1 thing left that was keeping him going and that was Ellie. He pretty much said it at the end of the game. Lol Joel showed marlene and her crew NO FUCKING MERCY when it came to Ellie. Even though it was probably right for Ellie to die for the sake of humanity, Joel didn't care. Joel became rhe bad guy at the end of the game.

 

Edit: also, that david guy was only eating people because they didn't have anymore food. There is a note on the counter right next to where you find Ellies backpack that pretty much shows how they were running extremely low on food. He was being nice and all buy Ellie was smart to not trust him because all he was trying to do was fuck her on some pedo type shit.

 

I knew joel wasn't perfect and has many flaws, it was obvious throughout the course of the game with everyone you met. I was ignoring these signs due to how he was with ellie and how the game emotionally manipulating us to sympathise with him "lost his daughter among other things." It was just during the hospital level it became so hard to ignore that and to see Joel's "true color."  He is not bad or evil, but he'll do whatever it takes to protect the ones he cares for. That what makes the last of us amazing. It leaves many things up to the interpretation of the players. It's morally ambiguous and filled with many shades of grey. We need more of that in storytelling of video games. Sure this kind of stories are hard to digest, but that what makes this game memorable and a modern classic. We can talk forever about the game and we won't end the discussion anytime soon.
