The most recent one for me, which is flimsy but irksome regardless, is from Watchdogs. You spend the entire game running to and from the ATM to withdraw cash that you hacked out of bank accounts. The game shows you pocket money. Then a story point has someone demanding cash and the response is along the lines of "I don't carry cash". Plot hole? Meh. Remarkably annoying regardless? Indeed.
