Sure, they could make it. But making it work? Games like Guitar Hero rely on the attachment fitting into the slot, and fitting flush against the side of the DS. An attachment would make that quite a bit more difficult.
And an attachment would make it more difficult to relay the rumble through into your hands.
It's quite the big deal. People don't want another peripheral, especially for a handheld. That's too bulky.
Again, as a few others have stated, making it smaller would be ridiculous. It's too small as is. The GBA Micro didn't sell well, and a DS Micro wouldn't either. What they need to do, rather, is to reshape the DS, making it sleeker and more comfortable in the hands. Then, give it larger buttons and larger screens, as well as a longer lasting battery, new screen options (touch screen sensitivity? A whole brightness bar (1-10 instead of 1-3)? There are a lot of possibilities), and a larger stylus... maybe even a second stylus slot.
