To use Playstation home as an example, I would say that PSHome was GRAND idea executed by a mind unfit for the task of bringing this GRAND idea to life as it should have been. So basically, Home is just a big ass pile of potential that Sony just sat on. They didn't know what the fuck they needed to do to bring that ambitious project to life as it was supposed to be.

I'm not sure if it's more of the fact that Home was simply just too ambitious for it's time or the fact that Sony didn't have the right mastermind behind the project.

A little bit of both.  Home needed 100% integration with the XMB and games.  I don't think game launching in Home was as robust as it needs to be.  Plus they just didn't/don't have the network infrastructure in place.  It took way too long to load areas.  It also doesn't help that they switched to the XMB for areas from the previous tiles.
