If this is faked there is some photoshopping going on the in the first pic. His calfs are smaller in the after, his biceps and triceps are much less flabby and are actually starting to show tone and his skin has tanned a bit.
I wouldn't be surprised if this was faked but if you look at the first pic and see no difference you're just not looking.
