Yeah its due to the sleeve it came in the North American version of the game should have had a proper case like the oversease verison of the game where each disc has it own spot. A quick fix that seems to work most of the time is to put the disc in very hot water for a short period of time. Neither of the copies that I have used had the 4th disc in a sleeve the first copy was a rental version of the game and the one have now I bought used near mint (very good case missing instruction book) and all the discs where stacked in the one spot where the fist three disc should be stacked. Bought it at Gamestop for $9.99 could have actually gotten it a bit cheaper and a slightly better copy in another store that sells used games that I visited later on the same day.
