Apart from a brief tutorial introducing the controls, the game is ridiculously hard and unforgiving at all times. There's thousands of ways to screw up. Even if you've played months on similar games and years on games in general, you will die over and over to start with.

People stick with it because it's incredibly rewarding to get things right and you can still have fun while getting 99% of everything wrong.

I'm not buying the argument that difficulty alone hurts sales. Being unfair or obtuse does.

This. I'm not a fan of permadeath in games, because I don't like having to replay a game from the start, but apart this extreme hard mode, I normally play in the other hardest modes available. And I almost always play a game in the hardest non-permadeath mode available since the start. I also hate extremely unbalanced bosses, I hated the first one in Heretic II, because having to fight him in a closed room with no cover,  beating him required twitch skills that were needed nowhere else in the game and that I haven't, so forcing me to cheat and use god mode to continue the game, I never use it and I hated having to do it that time.
