I think it was the RROD that by far has caused the current situation. Had the hardware been rock solid I really think the 360 would be at 20 million right now and the PS3 would be struggling at 6 or 7 million.
It's really too bad it happened because as far as software goes people like Peter Moore did an amazing job at getting games like Grand Theft Auto 4 and Resident Evil 5 for the system and gaining major support from a lot of developers that only gave token support at best to the original Xbox.
