Many people who got banned more than once in the month. They're usually short enough that a more troublesome poster can potentially rack up several per month (though eventually they grow in length to the point where that becomes impossible).

Sometimes we accidentally double-ban (two mods strike near-simultaneously), though Barozi usually doesn't count those as more than one.

Do the bans add up if that happens xD?

Say for an example Mod A issues a 3 day ban and Mod B issues a 2 day ban - would that make it a 5 day ban or does it go from what the last/recent moderation did?

They don't stack, no. The system tracks only from the last issuance of a ban, and the way the clock resets, it doesn't track down to the minute or to the hour, so if we banned at the wrong time of day, a one-day ban becomes a three or four hour ban.
