So did you set up an appointment with a doctor yet?
As an actual doctor I can say that some of the "self-help" advice in this thread are good (like distraction, physical excercise, meditation and even trying to get advice and encouragement from a good internet forum for the social anxiety and low self-esteem part ain't a bad idea), but you are in such a bad shape right now that you need to meet a psychatrist as soon as possible and start from there.
Obviously stay away from alcohol and non-prescribed drugs.
