People can "wake up" during actual surgery but because the drug suppresses all your bodily functions you can't communicate with the doctors. Basically you become aware of the surgery and you can feel the pain but you can't express it because the state of your body. You can't blink, you can't scream, you can't cry, etc.
In other words, its a complete nightmare. It's rare but it happens. If it does, you have a good claim for malpractice and pain and suffering.
