Very, very few games get an AO rating because no US retailer will carry an AO rated title nor will Sony or Nintendo approve licensing rights for an AO rated game on any of their consoles.  I think MS is the same.

 

Manhunt 2 for Wii was only the second time in history that a game was given the AO rating.  Rockstar made a few changes (soem screen blurring effect) that enabled it to obtain the M rating instead.  Even with the downgrade to M, many retailers still refused to carry it.

which btw (as i am american) - how much content is actually rated 18+? i thought like virtually no games got that rating (just manhunt and the like) or am i off on the European rating scale?
