It's not fetch quests. It's a ton of mini-scenarios. The game starts only with your tiny island, but you keep adding islands (and continents) to the world by completing those mini-scenarios (both in the past and present). It's like a lot of mini-rpgs connected to another - some of them pretty amazing, some of them just the usual (defeat the beast that's terrifying our village - but always with a twist - the beast is not what it looks like). In the end the world becomes huge.
There are more surprises, inclunding a big one near the end, but the game is mostly long because of those.
