Ohh, that's a hard one. I remember seeing some huge glitches that I couldn't believe long ago but I can't recall them.
There was one on Driver where I clipped a police car in a specific way and I ended up on top of a high ride building. There was also one on a Tomb Raider game where I fell through a wall or floor and ended up completing the level in a few minutes.
