It's a lot less restricted online, me and my friends have been buying certificate 18 games for years, online or through digital distribution. Most people under 18 shop with their parents too, so parents just buy it for them. People at car boot sales don't care either, they ignore age ratings and sell you DVDs and games based on the content they know is on there.
If somebody under 18 wants to buy a certificate 18 game, they'll get it. Some parents will make it harder for them than others, but they'll get it in the end.
Cigarettes and alcohol aren't sold online (or at least not to my knowledge), parents will be far less willing to buy it for their child and store restrictions are a lot tighter. Even then, kids still get cigarettes and alcohol through older siblings or others at school.
