Actually, the original use of the AAA system was 85 percent and up. Somehow, it's been relegated to the 90's, which is a bit unfair, imo, since a lot of "love it or hate it" games deserve AAA status.
Here is a list of what I consider AAA titles to be:
1. Receive largely positive reviews.
2. Improve the library of the system they are on, in a significant way.
3. Sell well, not necessarily great, but as well, or better than expected.
4. Seperate itself from other games in its genre.
